The following are required for ALL students:

    • Two measles (or MMR) immunizations (one after your first birthday and one at least one month later), OR
    • Documentation of positive measles titer (blood test), OR
    • Documentation of date of measles disease by your health care provider.

    • Acceptable TB tests:
      • Mantoux skin test: ONLY accepted if done in the USA because testing methods and interpretation may vary from country to country.
      • Quantiferon Gold or T-Spot (blood tests) are accepted from all countries. This is recommended if it is available in your country.

If you are unable to get a TB blood test in your country, you can get a TB test at Student Health Services during your orientation.

    • If your TB test is positive, or if you have had a positive TB test in the past, you will need a chest x-ray. We accept chest x-rays done within the past 6 months. The radiology report MUST be attached in order for us to accept the x-ray result.
  • Meningitis immunization - this is required for all students under 29 years old who will reside in University housing. You will not be allow to live in University housing if you have not received a vaccine that protects against the four subtypes of meningococcus - A, C, Y, and W-135.

The following are recommended for all students:

  • Proof of immunity to Varicella (Chicken Pox) will be required for all students beginning on August 1, 2010, but we recommend you meet this requirement now. This means you must provide proof of one of the following:
    • Two varicella immunizations, OR
    • Documentation of positive varicella titer, OR
    • Documentation of date of chicken pox disease by your health care provider.
  • Proof of immunity to Mumps will be required for all students beginning August 1, 2010, but we recommend you meet this requirement now. This means you must provide proof of one of the following:
    • Two mumps immunizations, OR
    • Documentation of positive mumps titer, OR
    • Documentation of date of mumps disease by your health care provider.
  • We recommend you be immunized for Hepatitis B. (This is not a requirement.)
    • Hepatitis B is a serious liver infection which may be asymptomatic; it is common in many countries of the world, especially countries in sub-Saharan Africa, in the Amazon basin of South America, and in countries in eastern and southeastern Asia. It is strongly recommended that all students (even those who have been immunized) who were born in any of these countries, or whose mothers were born in any of these countries, be screened for prior infection with a Hepatitis B surface antigen test. (CLICK HERE FOR INFORMATION ON HEPATITIS B)

It is very important that you complete the requirements and submit proof of immunizations on our Student Health History Form before your arrival at UConn.

Please complete and mail the form as soon as possible. Do not wait until your arrival on campus to return the completed form!

Be sure to keep copies of all your forms for your personal record.  If you have any other health and immunization documents, please make photocopies of everything and send them along with your health history form.  This will prevent having any problems with registration and housing when you arrive.

If you are not able to complete the requirements before your arrival at UConn, you will be provided with an opportunity to do so during your orientation program. Please note that this is not done free of charge. There will be fees associated with this service.
