Meningitis Vaccination Clinic
Connecticut state law requires meningitis vaccination for all students living in university-owned housing.
Students wishing to reside in University owned housing must submit one of the following:
- Verification by a physician of a meningitis vaccination within the last five years
(Health History Form that was sent to each entering student)
- Notification of a medical or religious exemption, or that the student is 29 years of age or older - Click Here for Exemption form
- The deadline for new students to submit the Meningitis Immunization Form is prior to moving into the residence halls. Check-in will not be permitted if you have not provided proof of immunization or requested an exemption.
To assist students in meeting this immunization requirement, the University of Connecticut Student Health Services will be sponsoring a meningitis vaccination clinic.
When/Where
- Friday, August 28th, 2009 from 9:00 a.m. - 3:00 p.m. at
Student Health Services
You must reserve a dose of vaccine prior to attending this clinic. Click Here to do so. Reservation deadline is August 19th.
Cost
- $160 (fee can be paid at the time of service or charged to your student fee bill)
What to bring
- Your UCONN Student ID Card
- Signed consent, if under 18 years of age, Click here for consent form
Your Health Insurance Card If you are covered by Anthem Blue Cross/Blue Shield, HealthNet, Aetna, TriCare or ConnectiCare or CIGNA, we will bill claims directly to your insurance. These are the only carriers that we will bill directly. Your insurance card must be presented at the clinic. For other insurance plans, please check with your insurance company regarding reimbursement and covered service.
